A fantastic opportunity for a positive and enthusiastic social worker who is looking for a career working in the Older Persons Team, working in a busy team your responsibilities will include: * Undertake assessments * Referral clients to required services * Keep accurate records * To promote the client's capacity for independence as far as possible. * To ensure that the client, as the subject of assessment, is enabled to exercise choice in the decision making regarding available services required to meet assessed need. * To plan appropriate service responses and exercise statutory powers as required. * To commission packages of care according to the outcome of assessment as agreed with the client concerned as far as possible and within available resources. The successful candidate must be a car driver will have a valid GSCC registration. As this is a temporary position valid UK working rights are essential.
