Working within a Community Team
Dealing with over 65's in crisis, doing assessments, equipment prescriptions and referrals
Working along side Intermediate Care Team
MUST have a car, no accommodation
Full time hours to be worked over 4 shifts. likely pattern would be:-
Day 1 - 8am-10pm
Day 2 - 8am - 8pm
Day 3 - 8am - 12pm
Day 4 - 8am - 4.30pm
(some days may be weekends)
