Job Description
The post is in a rapid response team that works in A&E and an acute admission unit with a focus on admission prevention.
The person should have experience with older people, particularly in falls and assessing mobility in relation to discharge. The work will require some follow up work in the community (1 - 2 visits max for some of the patients) so being a car driver is desirable, though not essential. Cars are provided.
The person will be well supported in a team that consists of a band 7 OT, band 7 nurse, social worker and OT technician (also available to the rest of the team). The person will also have access to support from physios in respiratory teams who work closely in the AAU environment. Support is also available from orthopaedic physios as appropriate. North London.
