Job Description

Qualified Social Worker required for a Surrey where you will be joining the Adults and Community Care Team.

As a Qualified Social Worker the main purpose of the role isTo ensure that the social care needs of individuals are met, within the eligibility policy of the department, by working in partnership with service users, their carers, professionals in other agencies, and service providers, to establish arrangements and services which make clear and measurable improvements to the quality of life of users and carers.

MAIN DUTIES AND RESPONSIBILITIES

Carry a caseload, of a volume and complexity which will be determined by the postholder's current and developing level of skill and experience, in discussion with the line manager

Ensure that the social care needs and welfare benefit entitlement of individuals in the community, or in hospital, are identified and met within the requirements of legislation and available resources

To co-ordinate a comprehensive holistic assessment of need and risk and develop care plans that focus on maximising the individual's independence and inclusion in the community. These may include the provision of Direct Payments

Ensure the provision of high quality services to individuals through creative, cost-effective care planning which identifies measurable outcomes for users and carers

To undertake Vulnerable Adults investigations as required

maintaining financial and administrative processes and records

Work in partnership with all internal and external professionals, involved in assessment and service provision, through effective communication and consultation networks, to ensure a co-ordinated response to users and carers

The Position is to start ASAP and is for 4 months initially. You must be eligible to work in the UK, hold a Social Work Qualification, GSCC registered with a current CRB, driver preferred.
