Job Description
Desirable London Borough is seeking a Qualified Social Worker – Adults, for a Hospital Discharge team to assist in the progression of an Adults & Families Hospital Service. You will require experience of working in either of the following: Physical Disabilities; Mental Health; Community Care; Vulnerable Adults; preferably Discharge but not essential; POVA cases.
Experience of working with Multi-disciplined teams will be considered an advantage, but not essential.
Job Description:
The Qualified Social Worker will be required to work with a multi-disciplined Adults team, assess and review caseloads in accordance to UK legislation.
Contract length: 6 months minimum, Ongoing!
