JOB TITTLE: Adults Mental Health Social Worker

MAIN PURPOSE OF THE JOB

1.Seconded to this London Borough and Mental Health Trust.

2.Provision of a social work service within the framework of the Care Programme Approach.

3.Working with individual service users in hospital and in the community providing advice, assistance, assessment, counselling, personal therapy or other forms of personal assistance with the aim of enhancing or protecting individual welfare.

DUTIES & RESPONSIBILITIES

1.Comply with the statutory requirements of the relevant childcare, mental health or community care legislation and in line with the requirements of the Council's policies and procedures.

2.Respond to requests for assistance, taking sufficient basic information about the needs in question to be able to determine the level and type of assessment required.

3.Maintain case records and fulfil procedures and statutory duties in line with national and local policies, practices and procedures.

4.Inform the line manager of specific needs that cannot be met because of the unavailability of services or because of inadequate budgetary provision.

5.Work within budgetary limits laid down by Council policy and operate within the Financial Regulations and Standing Orders of the Council.

6.Carry out assessment of needs, and sub-contract parts of such assessments where they are of a specialist nature.

7.Ensure that assessments pay particular attention to individual strengths and wishes.

8.Ensure that assessments take into account needs in all relevant user categories.

9.Undertake at all times to take into account the views of users and their carers, and to facilitate their participation in the assessment process.

10.Liaise with other specialists and agencies as necessary, and enable them to be involved and contribute to the assessment process.

11.Negotiate with users, carers and others concerning the methods to meet the needs identified by each assessment.

12.Ensure that care plans include clear statements of agreed aims for each service or resource to be provided.

13.Assist users and carers to play a full part in care planning by extending to them advice, support and guidance, and arranging where necessary access to translation, interpretation or advocacy services.

14.Review the progress of each care plan at regular, pre-determined intervals together with users, carers and other service providers to ensure the effectiveness of the plan.

15.Have effective liaison and working relationships with mental health agencies and service providers.

16.Attend case conferences, case meetings and team meetings as required.

17.Carry out responsibilities under mental health legislation and regulations including, where applicable, Approved Social Work duties.

18.Develop and implement plans aimed at resettling people with severe mental illness
