Job Description
We are seeking a Care manager to work within Adults Services. You will be required to work in an Adult assessment team with older persons.
You will be required to provide an assessment, care planning and review service for vulnerable adults in a hospital or community setting. You will manage a caseload involving users and carers at all stages in assessing, planning and reviewing their needs.
The successful applicant must be a qualified Social Worker, prepared to be flexible and maintain optimum levels of support at all times.
