Qualified Social Worker / Care manager required to work in a busy/friendly team who offer a care management services to inpatients over 18 years in an Acute General hospital. We require a hard working and highly motivated person who can deliver a quality service to patients and their carers and should have experience of working with people who have disabilities or older people in a care management setting preferably. It is essential you have good communication skills verbally and written.

The Position is to start ASAP and is for 3 months initially. You must be eligible to work in the UK, hold a Social Work Qualification, GSCC registered and have a current CRB.
