Job Description

Qualified Social Worker required to work for an East London Borough where you will join the Hospital Team. You will be working for a busy acute hospital as part of the local authorities hospital social work team. This will involve working with inpatients in a number of different acute hospital settings. You will be part of a busy hospital adult social work team which is focused on arranging safe and timely discharges of adult, mainly elderly patients. You will undertake assessments and produce care plans, arrange complex care packages and care placements as part of discharge planning and review these care packages and placements post discharge.

The Positions are to start ASAP and is for 3 months initially. You must be eligible to work in the UK, hold a Social Work Qualification, GSCC registered with a current CRB, driver preferred.
