Job Description

Qualified Social Worker required to work for to join hospital social work team in Central London. You will have experience in assessing clients over 18 in hospital settings as well as putting in place care packages reflecting clients needs and eligibility criteria. You will be expected to work as a duty SW few times per months, part of their duty will be to take initial assessment and screen the referrals as well as manage and answer any questions and queries. As a social worker you will be expected to manage case load of complex and straight forward hospital cases. You to have ability to respond to workload in relation to hospital discharge time frames and experience in performing hospital reviews in community and residential settings. You will also attend Multi Disciplinary and being part of the discharge planning process.

This Qualified Social Worker position is to start ASAP and is for 6 months initially. You must be eligible to work in the UK, hold a Social Work Qualification, GSCC registered with a current CRB, driver preferred. Applicants must be appropriately qualified at interview, have post-qualifying experience and especially experience of working with people with learning disabilities. (Currently, experience of Duty systems would also be helpful).
