Job Description

Qualified Social Worker required to work for a West London borough to join the Hospital Team. You will be working for a busy acute hospital as part of the local authorities hospital social work team. You will work with inpatients in a number of different acute hospital settings. You will be part of a busy hospital adult social work team which is focused on arranging safe and timely discharges of adult, mainly elderly, patients. You will participate pro-actively in the teams allocation process. You will undertake assessments and produce care plans. You will arrange complex care packages and care placements as part of discharge planning and review these care packages and placements post discharge. You will assess, record and manage risk. You will participate in a Duty cover rota on a weekly basis. You will progress investigations within the context of the Borough's Vulnerable Adult Protection Policy.

The Positions are to start ASAP and is for 6 months initially. You must be eligible to work in the UK, hold a Social Work Qualification, GSCC registered with a current CRB, driver preferred.
