Job Description

Qualified Social Worker -ASW- Provide professional social work service to residents of West London Borough experiencing mental health difficulties within the Council's eligibility criteria.

To manage a caseload, care coordinate care plans in line with the CPA Process, carry out assessments of need, assess/manage risk, keep up-to-date records.
Work as part of a team and forge close links with statutory and voluntary sectors.

Supervisory responsibility
To provide supervision of students and care-coordinators as and when appropriate.

Main responsibilities and tasks

1. Provide social work perspective on multi-disciplinary assessments of needs of service users and carers. Co-ordination of, monitoring/review of care plans and maintain accurate records including action and identification of unmet need. ASWs to make available specialist knowledge and expertise through advice, co-working and training and to contribute to mental health training within and outside the Borough.

2. To complete statutory forms as appropriate and provide social reports for
Tribunals, hearings, court etc.

3. To participate in costing; To meet service standards and performance
indicators as agreed with The Team Manager and Business Unit. Participate
in the production and implementation of the Business Plan

4. To participate in supervision, appraisal and training schemes to ensure an
effective service to service users and carers. To work with other agencies and services within an integrated Health and Social Services setting. ASWs to keep up to date on relevant policy and practice. ASWs to attend ASW and refresher training and re-warranting panels when appropriate, to maintain the warrant, and to attend the Borough's ASW forum to improve practice.

5. To provide duty cover as required; plan/manage crisis situations; participate in MDT meetings. ASWs to participate in ASW rota for statutory mental health assessments.

6. To contribute to and review service developments in respect of mental health policy and practice. Ensure that services are delivered in an anti-discriminatory way. To give information in respect of unmet need. ASWs to participate in the mental health monitoring system.

7. To keep up to date on policy and practice in mental health. ASWs to assist in meeting the requirements of mental health legislation, departmental policy and locally agreed procedures.

8.To make use of training opportunities to help own professional development.
