Job Description

The London Borough of Brent wants to recruit a Qualified Mental Health Social Worker.

The Qualified Social Worker will be required to work within the multi-disciplinary team structure of the Assertive Outreach Team, providing specialist professional advice or expertise in social care perspective and providing creative contact and support to service users who have severe and enduring mental health problems, and often disengage from services.

You will be required to carry a caseload and act as CPA Care Co-ordinator for service users allocated. In addition, the post-holder will be required to adopt a team approach (where roles may be blurred) and work with the other care co-ordinators to ensure effective team approaches to care. It is expected that the post-holder will make valuable contribution to multi-disciplinary team assessments; the formulation and implementation of MDT care plans.

The appointed Qualified Social Worker will be GSCC registered and have an enhanced CRB.
