Job Description

Social Workers are accountable to the Practice Manager for work with the individuals allocated to them, aimed at developing working relationships with agencies and organisations to promote service user independence in the local community, through the ESCC robust assessment and care management process. Making assessments of need and planning appropriate responses and services. Acting under appropriate legislation to safeguard the welfare of adults including providing accommodation in accordance with departmental policies and procedures: Safe Guarding Vulnerable Adults (SVA). Seeking to work in partnership with service users, carers, relatives and others to promote, develop and maintain a maximum quality of life. Providing information and advice to service users, cares and others. Arranging either directly or by referral the provision of additional services within a structured contractual framework. Monitoring the same. Maintaining records of their work as required in accordance with departmental policies and procedures.

In respect of the work of the team or area, there would also be a need for:

Attending meetings, the discussion of work policies and processes, allocations of resources, case conferencing etc. A commitment to continued training and professional development.
