Job Description

We are currently looking for a Qualified Social Worker to work within Community Services. The vacancy is based in Surrey within the referral and assessment team. You will be able to undertake work with clients who have mental health problems, to give them support whether they be in hospital or in the community. Work within a framework of the Council's Equal Opportunities Policy for Trainee Approved Social Worker posts. To participate in a process of assessment, and if successful, to undertake training to be an Approved Social Worker. Undertake casework with clients who have mental health problems and their families or carers. Make assessments of need under the Care Programme Approach in conjunction with the clients and carers in order to formulate care plans. Carry out care management duties in accordance with NHS and Community Care Act 1990. Provide the social work service to the client and carer involving both emotional and practical help. Attend multi-disciplinary meetings, such as ward rounds, liaison groups or case reviews. Undertake statutory duties under the Mental Health Act 1983 as required. Record the social work intervention in accordance with borough policies and the Data Protection Act 1984.

Candidates are required to hold a clear enhanced CRB, references covering the last 5 years and a full CV covering and explaining any gaps in your employment history and also having the reasons for leaving previous posts.

Candidates need to be registered with the General Social Care Council.
