Job Description

Main responsibilities and duties

Carrying a managed caseload and meets agreed workload targets.

Participating in team arrangements to provide an urgent or emergency response, during working hours, as required.

Undertaking assessments of user/carer need and constructs appropriate care plans.

Purchasing services to meet assessed needs, seeking the appropriate authorisation from the budget holder. Liaises with providers as required

Maintaining a knowledge and awareness of legislation and departmental policies and procedures, and works within these.

Working co-operatively with other professionals in the department, other Council departments, Health Service and voluntary sector.

Keeping both electronic and manual file records up to date using the department’s recording guidelines.

Candidates are required to hold a clear enhanced CRB, references covering the last 5 years and a full CV covering and explaining any gaps in your employment history and also having the reasons for leaving previous posts.

Candidates need to be registered with the General Social Care Council.
