Job Description

Candidates are required to hold a clear enhanced CRB, references covering the last 5 years and a full CV covering and explaining any gaps in your employment history and also having the reasons for leaving previous posts.

1. Work with service users, their carers and their advocates, where appropriate, in the assessment of individual needs and the provision of appropriate services/resources.

2. Identify the aims and objectives of proposed interventions

3. Identify unmet needs and to record on departmental information system.

4. Produce care plans and ensure their effective implementation.

5. Be involved in the direct management of certain care packages.

6. Manage within an agreed budget for the delivery of care.

7. Provide Social Work with homeless and potentially homeless persons/families

8. Undertake Approved Social Worker training and carry out Approved Social Worker duties

9. Act as Duty Officer

10. Assist in the induction of newly qualified/Approved Social Workers/Care Managers

11. Provide consultation to other Social Worker staff on a particular specification

12. Supervise Social Work students or trainees

13. Serve on ad hoc bodies/working parties

14. Record all work undertaken and prepare reports/assessments

15. Accept responsibility, with the Line Manager for receiving work on caseload at regular intervals. This would include closure and transfer of cases

16. Ensure that the quality of service provided meets agreed quality standards.

17. Maintain effective contracts with all providers of care and other people responsible for relevant community resources.

Candidates need to be registered with the General Social Care Council.
