Job summary

JOB PURPOSE

The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.

The post holder will be required to: Work on a rotational shift basis within the Brent Rapid Response Team – service operating hours 8am – 10:30 pm 7 day a week.
